Although the state mandate has been lifted as of March 01, 2021, the guidelines and safety protocols regarding Covid-19 remain the same, and Blue Diamond Body and Skin will continue to require all guest procedures as outlined below. Even if you have received your full vaccine, the same requirements will still apply to receive services at our facility. We recognize and wholeheartedly agree that the life is not as convenient as it used to be, but we genuinely appreciate your willingness to help us make sure that the risk to our community is minimized as much as possible. Intake Forms Required for All Guests:

1. Confidential Health Questionnaire, prior to first appointment, to be updated once per year.

   Emailed after appointment reservation. Please submit no later than the day before appointment.

   The updated form is required for all guests, even if the older version was completed within the last 12 months.

2. Initial Session Intake. Skincare, Waxing, and Massage & Bodywork each require a separate form.

   Emailed after appointment reservation. Please submit no later than the day before appointment.

3. COVID-19 Intake Form, required before every appointment until the health crises has passed.

   Emailed to you after appointment reservation. Please complete and submit asap.

4. UPDATE - Covid-19 & Consent for Treatment, prior to every treatment after the initial appointment.

To maintain 6-foot social distancing and to minimize possible transmission of contagions as much as possible, please do the following:

 

◊ All guests are required to wear a mask/face covering to enter our facility and to keep the mask on during treatment as services permit. (If you are receiving a facial or lip/chin wax, your mask may be removed during the treatment but must be worn before and after.) The mask must completely cover from your nose to chin. Please ensure proper fitting of your mask, so that it can provide proper protection and is not falling from your nose and mouth while speaking. The Spa does not provide guest masks. No services will be performed if you arrive without a mask, but you will be responsible for payment of the scheduled time. Please ensure that you have your mask before arrival.

 

◊ Text or call (501) 350-0488 to let your therapist know you have arrived.

 

◊ Remain outside in your vehicle until your therapist comes for you. Please do not stand at the door while waiting.

 

◊ No one who does not have an appointment will be allowed inside. This includes children; if they do not have a scheduled service, they cannot enter the facility. (If a minor is receiving a service, a parent or legal guardian is required to be in the treatment room.)

 

◊ Your therapist will ask you the in-person COVID-19 Screening questions.

 

◊ Allow your therapist to open the door for you, both at the front as well as treatment room doors.

 

◊ Refrain from touching items such as retail products and décor items. Your therapist will be happy to assist with products.

 

◊ Wash your hands for at least 20 seconds prior to entering the treatment room.

 

◊ If paying with a debit or credit card, we have a hands free card reader, but you might consider creating an account on our app and placing your card on file, or using Apple Pay / Google Pay and simply tap your phone to the payment terminal at the time of checkout.

 

IMPORTANT:

Services will not be provided for anyone who is sick or showing signs of illness, including a head cold and/or allergy symptoms. If you, or a member of your household or someone you spend time with, is sick or showing signs of illness, please contact us immediately to reschedule your appointment. There are no cancellation fees for sickness. If you choose to knowingly arrive sick or with symptoms, including allergies, the appointment will be cancelled, and you will be responsible for paying the full amount of the services scheduled that day.

If you have been exposed to someone known or suspected to be infected with Covid-19, whether it is a job-related or personal exposure, we cannot provide services until you have been free of all symptoms of illness for at least 14 days from the date of last exposure.

If you, or someone you live with or spend time with, has traveled out of Arkansas (domestically or internationally) or has traveled within the state to a location with or near a high infection rate (a "hot spot"), we cannot provide services until you have been home and free of all symptoms of illness for at least 14 days. If you are visiting the state of Arkansas, we cannot provide services until you have been here and free of all symptoms of illness for at least 14 days.

​RESCHEDULING/CANCELLATION/FAILURE TO SHOW FEES - Non-Emergency Reasons​

Up to 24 hours before your appointment time - no fee

Less than 24 hours - $25 late cancellation fee

Less than 12 hours - 50% of the total amount for the scheduled services

Failure to Show - 100%  of the total amount for the scheduled services
